Student Enrollment Information

 
1.  Registration packets are available at the front office.                                            
 
2.  Please bring the following items with you when you register your student(s):
  • Birth certificate with State seal
  • Physical examination form (Kindergarten and first grade only)
  • Dental exam form (Kindergarten and first grade only)
  • Immunization records (yellow immunization card)
  • Two proofs of residency (current utility bill, rental receipt, lease agreement, escrow papers)
  • Last report card (for placement purposes)
3.  Registration hours will be from 9:00 a.m. to 2:00 p.m., Monday through Friday.
